What to Expect

What to bring to your office visit

​

  • Current insurance card

  • A list of all medications you are currently taking, including prescriptions, over-the-counter drugs, vitamins & supplements. (You may bring the containers with you instead of a list, if you wish.)

  • Copies of any records you may have from previous doctors including lab results, test results, or letters

  • Glucose meter if you have one

  • Completed information forms requested by the physician

  • Personal identification (driver’s license is fine)

  • Method of payment for services not covered by insurance i.e. cash, check credit card (MasterCard, Visa, American Express etc.)

Keeping helpful information about your visit in the Electronic Medical Record (EMR)

​

Your physician provides management of acute and chronic illnesses, health maintenance, immunizations, and treatment of minor injuries and common complaints.

​

The following information will be updated in the electronic medical record at your visit:

​

  • Vital signs

  • Current medications

  • Allergies

  • Smoking status

  • Reason for your visit

​

The physician or medical extender will then continue your exam. He or she will order any tests, labs, or x-rays necessary, and schedule you appropriately.
